Assistance for stable and secure housing situations

Housing Stabilization Services is a program supported by Medical Assistance, designed to assist people with disabilities, including those with mental health issues and substance use disorders, as well as older adults, in securing and sustaining housing. Stable and affordable housing is crucial for the well-being of these individuals. The goal of the service is to facilitate a smooth transition into housing for individuals, enhancing their long-term stability in their living situation within the community, and preventing future instances of homelessness or the need for institutional care.

HSS stages

Consultation Stage – Our team will work with eligible individuals who do not have waiver services create a person-centered housing plan. We will help the person to select their housing transition or sustaining provider.

Transition Stage – Our HSS team will assist eligible individuals find a home of their own. We will support the person in identifying and overcoming barriers and assist with the housing search process. Upon finding a home, we will assist with coordinating the move.

Sustaining Stage – Our team is dedicated to helping individuals maintain housing and prevent homelessness. We may support the person with budgeting, paying rent, and identifying community resources. Our goal is to support the person with maintaining long-term housing stability.
